The Pathway 360 Assessment™ is an industry-leading home modification assessment that documents risks and opportunities by area of the home. The Pathway 360 Assessment™ is captured on our customer mobile application by our certified construction provider and includes detailed documentation and photographs before and after a project is completed.
Pathway Certified Construction Provider™ (PCCP)
The PCCP is a certification awarded to construction providers that pass the Pathway certification process. Certification application criteria include active licensure, project history, and references. Construction providers begin the certification application by submitting this form.
Communications
We provide a single point of communication for health plans, members, and construction providers through our health plan dashboard, text, phone, and email.
